WE NEED YOUR EMERGENCY CONTACT INFO...

HOW CAN WE CONTACT YOU ABOUT AN EMERGENCY?

There have been many times this past year in our neighborhood where the need has arose to get in touch with an HSCW resident in an emergency situation. However, since some of you haven't turned in your surveys, I didn't have the information needed to get in touch with some of you. So, please fill our your survey TODAY and get it back to me.

SURVEY INSTRUCTIONS

The information on this survey will be used to protect our area from criminal activities, make us aware of who belongs here and some of it will be added to our Neighborhood Phone Directory. After you return this survey to your Block Captain, it will be forwarded to the Chairman of our Crime Watch, Cindy Green, who will hold all originals in her possession. If there is any information that you don't want listed in the directory, please list it anyway and place a big "X" next to the information you want left out. All information is conpletely CONFIDENTIAL and will not be given to anyone unless we have your approval. Please fill out the survey below and return it to your Block Captain immediately. We are always in need of donations since our group is on a non-paid volunteer basis only. Everyone who participates are on a volunteer basis only and don't receive any compensation for their time. All donations are for paper for our copies, printing services for our copies, tape to hang notices on your doors, ink for home printing of smaller notices, pens, materials for signs, etc. If you are able to give anything, please send it to the following address.
