North Carolina Absentee Ballot Portal (ncsbe.gov) Use the NCBOE link to make the request. To obtain ballot you must be registered. Pretty simple process but you must provide DL# or last 4 of social security during the process.

When you send your ballot in you will need a copy of your DL to insert on the ballot envelope before sealing. So be prepared to make a copy if you don't have a copy machine.
Ballots for Absentee require two witnesses or one notary signature.
They need to be present when you fill out your ballot. They will then sign the envelope as the witnesses.
