Dear Alum:

e are currently seeking nominations for the 14^th Annual Alumni Awards Celebration, which will be held in the fall of this year. As a member of the CTC/CSU Alumni Association, you are eligible to nominate alumni, staff or faculty of Chicago State University to receive these prestigious awards.

Over the last 13 years, alumni have nominated individuals whose personal accomplishments and contributions to the local and global community reflect CSU’s proud 148-year tradition of academic excellence and its mission to educate, empower, and serve. At last year’s celebration, the Alumni Association celebrated with guests and provided a great avenue for alumni to connect and celebrate our community’s accomplishments in the fields of business, education, public and community service, health sciences, and the arts.

Help us make this year’s program even better by using the attached nomination form to recommend outstanding alumni and other individuals who have provided exceptional support for CSU. Please note that all nominations must be submitted to the Office of Alumni Affairs by Tuesday, June 30, 2015.
