Kindergarten Round-up

Greenwood Hills Elementary is having it annual Kindergarten Round-up on Thursday, April 11 from 6 to 7:30 p.m.

If you have a child that will be five (5) years old on or before September 1, 2002.

The following information is required:

 Birth certificate (for students aged 10 and under)
 Your child's Social Security Card
 Proof of residency (utility bill for homeowners or apartment lease with the parent's name on the lease if you live in an apartment)
 Proof of withdrawal from your child's former school
 Proof of the following immunizations (please bring all shot records to the school nurse):

The following completed and up-to-date immunizations are necessary for school enrollment and continued attendance:

 Diphtheria/Tetanus/Pertussis:

Every student under 5 years of age must enter with a minimum of 4 doses.

Every student 5-6 years must enter with a minimum of 4 doses, with at least one dose given after the 4th birthday.

Every student 7 years or older must enter with a minimum of 3 doses, with at least one dose given after the 4th birthday.

A booster is required every 10 years.

Pertussis vaccine is not required for students 5 years of age and older.

 Polio:

Every student under 5 years of age must enter with a minimum of 3 doses.

Every student 5 years or older must enter with a minimum of 3 doses, with at least one dose given after the 4th birthday.

Polio vaccine not required for persons 18 years of age and older.

 Measles:

Requirement is first dose on or after the 1st birthday.

Students who are 5 years of age or older and who were born on or after September 2, 1991 must enter with two measles doses.

Students born prior to September 2, 1991 must have a second measles vaccination by the 12th birthday.

Only serologic confirmation (laboratory report) of immunity in lieu of vaccine is acceptable.

 Rubella:

Requirement is one dose on or after the 1st birthday.
Only serologic confirmation (laboratory report) of immunity in lieu of vaccine is acceptable.

 Mumps:

Requirement is one dose on or after the 1st birthday.
Only serologic confirmation (laboratory report) of immunity in lieu of vaccine is acceptable.

 Haemophilus Influenzae Type b (Hib):

Requirement is one dose for ages 15 months through 4 years, unless a schedule for a primary series and booster was met by 15 months of age.

Not required after 5 years of age.
Physician-validated date of invasive Hib disease after 2nd birthday will substitute for vaccine requirement.

 Hepatitis B:

Requirement is three doses for students who are 5 years of age or older and who were born on or after
September 2, 1992. Only serologic confirmation is acceptable in lieu of vaccination.

Students born on or after September 2, 1988 but before September 2, 1992 will require 3 doses of vaccine or serologic confirmation of immunity by the time of the 12th birthday.

 Varicella (chickenpox):

Students born on or after September 2, 1994 will require either:
One dose of vaccine on or after the 1st birthday, or
Written validation from the parent or physician giving the approximate date of the varicella illness, or

 Serologic confirmation of varicella immunity.

Students born on or after September 2, 1988, but before September 2, 1994 will require by their 12th birthday proof of either:

One dose of vaccine on or after the 1st birthday, or
Written validation from the parent or physician giving the approximate date of the varicella illness, or
Serologic confirmation of varicella immunity.
